- List of added scope of practice, job responsibilities, and/or CE’s since the last time they were graded/audited that does NOT reflect your current job description/scope of service
- Any wage scale chart you are able to obtain from other Hawaii Market facilities (e.g. QMC, HPH or Tripler)
- Utilizing this information please write a testimonial to present to the Employer why this classification should be considered for an upgrade.
