MEMBERS: Important information on your prescription with Mina Pharmacy

You may be receiving the following letter re: the recent news that Mina Pharmacy has been sold to CVS/Longs. Please contact the Local 5 office at (808) 941-2141 if you have any questions or concerns.

Dear Valued Participant:

As your pharmacy benefits manager, Catamaran, in collaboration with AFL Hotel and Restaurant Workers Health and Welfare Trust Fund, would like to take this opportunity to inform you of upcoming changes to your pharmacy network that will go into effect on April 11, 2015.

Mina Pharmacies has been sold to CVS/Longs. As you know, CVS/Longs is not a participating pharmacy chain in your pharmacy network. Effective April 11, 2015, you will no longer be able to obtain your medication at this location.

You will need to transfer your prescription(s) over to a participating pharmacy. There are a couple of ways to get this accomplished:

  • Pharmacy Assistance: a) Ask the participating pharmacy of your choice to call CVS/Longs to transfer the prescription to them
  • Prescriber Assistance: Contact your prescribing physician to request new prescriptions called in to the participating pharmacy of your choice.

We are providing you with notice of this change in an effort to ensure that there is minimal disruption to you and your prescription drug therapy.
